It had been a busy day in OPD. My last patient was a middle aged lady with Rheumatoid arthritis. She had been pain free for the last 1 year and was happy with the treatment. But she had a query: sir a nearby doctor repeated my uric acid and it's still high. Why is it not coming down despite treatment ! ?? This happens time and again..

Uric acid is the by-product of purine metabolism. Purines are produced in our body when there is cell breakdown or tissue damage. External sources include animal proteins like seafood, red meat or organ meat like liver. Another common source is fructose containing artificially sweetened soft drinks and beer. Any excess of Uric acid is thrown out of the body through the kidneys. That's how the body maintains normal uric acid levels.

Whenever there is an imbalance in the above processes, like excessive intake, injury or blood cancer or if the kidneys are not functioning properly, uric acid levels increase in the body.

Uric acid crystals get deposited in joints and cause very severe joint pain and swelling. This is called acute gout (commonly called uric acid ki gathiya) and is described by the patient as the most severe pain he has had in life. The pain almost always resolves within a week with or without any treatment. Almost 80-90 % of the time, it involves the big toe. If treatment is not started, patients may develop uric acid deposits (Tophi). This is called tophaceous gout. High uric acid can also cause high Blood pressure and kidney damage.

Increased uric acid without any problems usually does not need any treatment. However, if the level is very high or if the above mentioned problems occur, treatment needs to be started.
