Which Custom Abutment and Why

by William Sam Consulatant
When you get a tooth implant, it can have three distinct parts that go together to make it work – the implant itself that is embedded into the jaw bone, the custom-made crown which goes in the place of your enamel and the custom abutment in between the two right where the gum tissue is present.

Which Custom Abutment and Why?

Custom abutment in your teeth is the part of that holds your implant and the new crown together. While using a custom abutment is not compulsory in an implant, it is necessary in many cases.

  • When the patient needs a full mouth dental bridge implant
  • When the patient has too thick or too thin gums
  • When the implanted tooth is one of the incisors or canines
  • When the patient has over-dentures, and they have a bar to hold them in place
  • When the space between the patient’s upper and lower teeth is really small, and cement is not enough to hold the implant in place

Next, we come to the different types of custom abutments that are available and which one is applicable in which case:

1. Titanium custom abutments: Titanium is the most common form of abutment used by dentists all over the world. They are cheaper than the rest of the abutment types and also strong and durable. The best thing about titanium custom abutments is that they can easily be used in any part of the dental crown. They are the easiest to set up in the mouth – one of the many reasons behind their popularity among the dental surgeons.

2. Zirconia custom abutments: Having an all-ceramic composition, zirconia custom abutments are the first preference it aesthetics are the priority. Because of its colorless body, they blend in perfectly with the usual shade of the gingiva. That is why these are used in the implants of the incisors so that the steel shade of the titanium does not show through the gum. Zirconia abutments are thus used in the front of the crown, or what is technically termed as anterior of the maxillary arch. Over and above aesthetics, zirconia abutments are also more hygienic than titanium ones.

3. Zirconia custom abutments with a titanium connection: These abutments, commonly called zirconia hybrid abutments have the aesthetics of zirconia and the convenience of titanium. A titanium base is given to an abutment of zirconia, so that provide titanium to titanium installation to the implant in your bone. Because titanium is the more common and easily attached material, it takes lesser difficulty than zirconia. But because of the zirconia covering, the titanium does not show through the gum, even if the transplant is done in the anterior of the maxillary arch. So if you go with these hybrid abutments, you kill two birds with one stone.

The choice of prosthetics depends on what the dentist suggests, but at the end the decision is yours. Choose wisely, knowing what is more important to you – ease and convenience, or beauty and aesthetics.
