Heart surgery patients are typically given general anesthesia before open heart surgery; which means that they will be unconscious (asleep) & pain-free during the operation. The cardiac surgeon will than make an 8 – 10 inch surgical incision in the middle of the patients’ chest at the start of open heart surgery procedure. The breastbone is subsequently separated in order to create an opening. This is to allow surgeons to approach the heart & aorta which is the main blood vessel leading from heart & connecting with the rest of the body.

Operating Steps during Open Heart CABG Surgery

Most people undergoing CABG surgery are connected with a bypass pump or a heart-lung bypass machine.


The heart is now stopped while the patient is connected to the heart-lung bypass machine. The heart-lung bypass machine will take over & perform the work of the heart & lungs while the heart is stopped for the purpose of operation. Throughout the heart surgery, the heart-lung bypass machine will add oxygen to blood & make it flow through the body & remove carbon dioxide at the same time.

However, there is another type of CABG surgery which does not make use of the heart-lung bypass machine. This procedure is performed while the heart is still beating & is known as the OPCAB or off pump CABG.

Graft Blood Vessel for CABG Surgery

Bypass graft vessels have to be harvested for which cardiac surgeons take the following steps:

  • Surgeons will take a portion of an artery or vein from another part of the patient’s body & utilize it to make a detour (bypass) around the blocked portion of the coronary artery.

  • The surgeon may use a vein known as saphenous vein extracted from the leg, for CABG procedure.

  • A surgical incision may be made along the inside of leg so as to reach the saphenous vein any place between the ankle & groin. One end of the CABG graft will then be sewn to the coronary artery & the other will be sewn to an opening made in the aorta.

  • Blood vessel in the chest known as the internal mammary artery is also in some cases used as graft for CABG procedure. Since one end of this artery is already connected to a branch of aorta, only the other end has to be attached to the coronary artery.

  • Some other arteries can also be used as grafts for CABG procedure. Most common among these is the radial artery in the wrist area.

After the graft vessel for CABG is created, breastbone will subsequently be closed with wires which will remain inside. After this the surgical incision will be stitched close. CABG open heart surgery can last anywhere between 4 – 6 hours of time. After completion of the open heart surgery, the patient will be taken to the intensive care unit (ICU) for recovery & where he will be closely monitored.
