Surgical Sperm Collection (MESA, TESA & PESA)

MESA, TESA & PESA are modern specialized technique which is used in conjunction with the sections on In Vitro Fertilization (IVF) and Intra Cytoplasmic Sperm Microinjection (ICSI). Before explaining all these technique, let’s have an overview of the male reproductive system.

Human reproductive system is very much complex to understand whether it is of male or female. Male reproductive system is mainly for producing, storing and transporting the sperm outside the body.

The organ that produces sperm is testes which only produce immature sperm. These sperm travel along with the epididymis where they become mature and motile. At the time of ejaculation, these are carried from the epididymis to the penis.
While there were several treatments available for female infertility, there was no treatment for men who have no sperm in the azoospermia and as per the estimation azoospermia is the cause of male infertility in about 10-15 % of fertility cases. There are many causes of azoospermia such as obstructive where there is a blockage in the delivering of sperm. Other causes include absence of sperm or very less production of sperm in the testes. It is always advisable to review by urologist if you are struggling from azoospermia.

Surgical Sperm Collection

The surgical sperm collection process is often recommended for male infertility cause due to low sperm count, low sperm motility and sperm abnormalities. Generally there are two methods of surgically sperm retrieving from the male testes and they are:

MESA (Micro Epididymal Sperm Aspiration)

MESA is not a new technique but a modern way to retrieve the sperm from male testes. Through the scrotal skin, an incision is made and with the help of optimal magnification, tiny tubules can be incised and aspirate the sperm rich fluid. Millions of motile sperm is retrieved and used freshly for ICSI procedure where a single sperm is injected into an egg to achieve pregnancy. The retrieved sperm can be sometimes frozen for future IVF (In Vitro Fertilization) or ICSI (Intracytoplasmic Sperm Injection).

TESA (Testicular Sperm Aspiration)

Aspirating a small piece of tissue from the testis and isolating the sperm from the tubules is involved in TESA. Through the scrotal skin into the testicle, a needle is passed through which a very small amount of testicular tissue is retrieved. Most often it is usually adequate in case of obstruction for IVF or ICSI but not for frozen process. However in case of poor sperm production, the same technique may be performed via multiple entry points in the testicle.

Precutaneous Epididymal Sperm Aspiration (PESA)


PESA is another technique used for the same where a needle is passed into the epididymis to retrieve the sperm. The epididymal tubule of male reproductive system is very complicated and delicate part and this is the reason the needle which is used is very small and used only one time for a single IVF- ICSI cycle.
