Efficacy of Dostarlimab in Treating Cancer and Its Effect Along With the Ayurvedic View

ABSTRACT

Monoclonal antibodies revolutionised cancer immunotherapy by blocking the interaction between programmed cell death 1(PD-1) and its ligand (PD-L1).  However it's been a challenging task for so many years to make these drugs efficient and safe for use and manage their immunogenic response .The immunogenic potential of Dostarlimab is not known properly and it was therefore necessary to analyse and detect its adverse reaction. This is discovered after so much research but still there are chances of its side effects on some individuals.

It is the only PD 1 immunotherapy that is clinically tested and effective in endometrial cancer. Endometrial carcinoma is the most common type of cancer of the female genital tract. Endometrial cancer is the four most common types of cancer ranking after breast cancer, lung and bowel cancer. Overall about 2-3% of women developed cancer in their lifetime. In ayurveda many herbal medicines are discussed which can be helpful in treating patients with tumors, cancers and they have no side effects . In this article we discussed dostarlimab drugs and also ayurvedic medicine which can be helpful in fighting against cancer.

INTRODUCTION

Prior to discussing dostarlimab let's discuss cancer and its signs and symptoms.Cancer is the uncontrolled growth of normal cells that expands locally by invasion and systematically by metastasis.

DOSTARLIMAB

Dostarlimab as we mentioned earlier is an  approved monoclonal antibody(Ig4) developed against many types of cancer. This drug was associated with a low risk of eliciting clinically meaningful adverse drug reactions and it is already approved by health authorities. In the USA and Europe it was approved recently. Because it is a macromolecular therapeutic, it has the potential to elicit the formation of antidrug antibodies, which are capable of working efficiently and safely. But still this is challenging because of its widespread use in many patients. One is the immunogenicity of the patient when administered with this drug. Development of anti drug antibodies varies in every patient, dose, drug of administration.

FUNCTION OF DOSTARLIMAB

It is an anti -PD 1(PD1 are proteins present on the T cell, T cell works to kill the cancer cells and these proteins act like brakes to T cell , therefore for proper functioning of t cell drugs are needed to act on PD 1 )which has a very high affinity to bind with the pd 1 so that it helps in killing cancer cells. Agents that are acting on PD 1 pathway such as nivolumab and pembrolizumab are acting as endogenous immune mediated antitumor activity and because of this they can be used in various kinds of cancer like skin, lung, kidney and liver.

1. Cough, chest pain or breathlessness
2. Dark coloured urine, yellowing of skin or eyes, bleeding or bruises
3. Headache
4. Deepening of voice
5. Weakness
6. Hair loss
7. Dizziness
8. Increased sweating
9. Fast heartbeat
10. Rash- skin blisters, peeling, itching, skin lumps etc
    
    

AYURVEDIC VIEW

Cancer is one of the deadliest diseases and challenges for medical sciences but the facts says that cancer is related to modernization and stressed life dominated by allopathy disease. Scientists are making many efforts to cure this disease properly but the cure with full efficacy and safety is still awaited

Ayurveda is the oldest medicine system of plant drugs from many centuries and natural drugs have been used for many years to treat this disease.

What is Cancer (Ayurveda)?

According to Acharya Charak and Sushruta, cancer is inflammatory or non- inflammatory swelling correlated with granthi( minor neoplasm) and arburda( major neoplasm).  Tridoshas vitiation is considered to be the main cause as per ayurveda. 2. Bhoomi amla (Phyllanthus niruri)

Phytochemicals that are present in phyllanthus niruri which have many properties are as follows: lignans which are hepatoprotective, terpenes exhibit antimicrobial activity and flavonoids useful against  inflammation and have antioxidant properties also. It is useful as antimutagenic, antitumour, antioxidant and antihyperuricemic.

3. Pippali (Piper longum)

It is known to kill cancerous cells in many tumors including brain tumors. It acts in cancer by binding and hindering the activity of protein TRPV2 , which is overexpressed in glioblastoma(brain cancer) , which enhances cancer progress.

4. Ashwagandha (Withania somnifera)

Ashwagandha roots are very helpful in treating many types of cancer. Its main components withanolides and withaferin are effective against different types of cancer cell lines.

5. Mahua (Madhuca indica)

It is a plant of Indian origin and has many therapeutic properties but does not utilize properly due to unawareness. It is useful in treating various diseases like - tuberculosis, rheumatoid arthritis, cholera, paralysis, and antidotes for poison. It is said that the bark of a mahua tree is very useful in treating cancer.
