An Electronic Health Record (EHR) is an electronic adaptation of a patient's clinical chronicle, that is kept up by the provider over periods of time, and usually includes all of the fundamental administrative medical information relevant to that person's care under a specific health care provider, including demographics, progress reports, diseases suffered from, diagnosis, vital signs, past medical history, vaccinations, laboratory data and radiology accounts. These standards stress the use of National UID or AADHAR number as the firsthand Unique Health Identifier (UHID) of a patient seeing a healthcare provider. The basic idea of the EHR standards in India is to have a country wide rollout of Electronic Health Records for all healthcare set ups and connect it to the AADHAR of the patient.

Privacy and secrecy is also a significant element. Using EHR software can possibly put an organization at risk if it does not follow the privacy guidelines to an exacting degree.
