What is cervical spondylosis?

As we age a process of wear and tear occurs in our whole body.

The same wear and tear and ageing also occurs in the cervical spine. These changes include degeneration of disc in the neck, herniation of cervical disc and also bony overgrowth in the cervical vertebrae known as osteophytes.

These changes in the cervical spine which occur due to normal ageing process of the cervical spine are called cervical spondylosis.

Which patients are affected by cervical spondylosis?

Usually the features of cervical spondylosis and resultant symptoms occur after the age of 40 to 50 years and more so after 60 years of age, but these days due to increase in sedentary lifestyle and also greater work on the computers and laptops, it is commonly seen in youngsters nowadays.

What are the symptoms of cervical spondylosis?

Cervical spondylosis can cause pressure over nerves and may result into following symptoms:

  • Neck pain
  • Pain, radiating to both the shoulders.
  • Pain in the lower part of back of head also known as occipital pain.
  • Pain, radiating down to one or both arm and forearm also known as cervical radiculopathy.

What is cervical radiculopathy?

Sometimes due to changes in the cervical spine like cervical spondylosis the osteophytes or the degenerated desk or herniated disc impinges and compress over the nerve roots.

In these cases pain, radiates down one upper limbs over the arm and forearm. This radiation of pain is called cervical radiculopathy.

The symptoms may also include tingling and numbness, paraesthesias, pins and needle over the affected area.

Rarely there maybe weakness of one arm or hand. The most common levels involved are C5/ C6 and C6/ C7.

The treatment of cervical spondylosis and cervical radiculopathy is mainly conservative, that is medical management is usually sufficient.

The treatment part includes some medicines which include analgesics, muscle, relaxants and nerve pain medicines. The treatment part also includes physiotherapy, hot fermentation and regular neck and shoulder exercises.

The patient is also encouraged to adopt healthy lifestyle which includes daily walking or yoga for 30 minutes, various yoga and other forms of exercises to maintain overall body health. Patients are also encouraged to stop smoking, excessive alcohol intake and to reduce weight.

Most of the time these measures suffice and the symptoms of cervical pain are relieved.

Most important thing to remember is that the cervical pain is an episodic and intermittent disease, which means that the symptoms of cervical spondylosis or not permanent, they come and go, they may come remain for 15 to 20 days or one month and then become better after treatment. When is cervical spine surgery needed?

Majority of the patients of above mentioned symptoms get relieved by medical management only. But sometimes the symptoms do not improve. These Patients may need surgery.

Patients with herniated cervical disc will need surgery in the form of anterior cervical Dissectomy and fusion also known as ACDF.

The ACDF surgery is done from the front of the neck. In this surgery, the prolapsed disc material is removed and the empty space is filled with Bone Graft and it is fixed by applying a titanium plate with screws for stabilization.

The goal of the surgery is creation of the bone in the space left behind after taking out the disc.

Sometimes another form of surgery known as cervical laminectomy or cervical laminoplasty is required.

Another form of surgery is cervical disc replacement.
