As an admin of my own text ad exchange, I approve all ads submitted by the entire membership of my exchange. The main items I look for is:

1. Does it break frames?
2. Does it contain inappropriate content?
3. Does the link work?
4. Is it a out right scam?

As long as the advertiser does not submit something that breaks the terms of service and it doesn't have any of the problems listed above, I approve the ads for the advertiser. In my normal daily routine of approving ads, I see almost everything that is being promoted online today. The one mistake I almost always see is that the advertiser is not building their list.

Always Build Your List First!

If you are simply promoting your standard affiliate or referral link, you are wasting your time and your advertising will not produce the best results. You must build your list first by using a lead capture page. Doing so will enable you to build your list before you send your prospect to the main affiliate page. If they don't sign up and you haven't used a lead capture page, you have no chance of communicating with those prospects ever again. The moment has passed. If you don't use a capture page and they do sign up, you have effectively built the owners list for them and not yours.
