Depends on you, if the artist can create something completely new out of it, it depends on its creativity. And if you want it to be completely removed then you can do a laser tattoo removal. It will get dissolve completely after a few weeks.

Yes it can be, talk to your artist about it. If he is not help look for more artists in your area who can help you either with a cover up on the original tattoo or by fixing the originally scarred one.

If it looks very scarred also consult a doctor to see if you have any skin condition or allergy due to the ink used in the tattoo.
