Below mentioned documents are compulsory for Transporting Pet from India to USA

Blood tests
Vaccinations
Microchips for identification
Health certificates or “Certificates of Veterinary Inspection”

Vaccination certificate must include below mentioned details. Vaccination is not required when you are importing your pet from rabies free countries

Name and address of owner
Breed, sex, age, color, markings, and other identifying information for the dog
Date of rabies vaccination and vaccine product information
Date the vaccination expires
Name, license number, address, and signature of veterinarian

International airlines will not allow pets inside cabin. You can transport your pet either by accompanied baggage or cargo. Some airlines do not allow snub nosed dogs like bulldog, boxer etc to fly, because of breathing issues at high attitude. Snub nosed cats are also not allowed to fly.

How to select crate for your pet:
Crate should be properly ventilated for pet to breath. Pet should be able to sit erect, stand, turn and lie in a natural position inside the crate. Below is the crate size prescribed by IATA.
