Ratification of notarized and public documents

The apostille is a printed form consisting of 10 numbered standard fields. On the top is the word APOSTILLE, under the “Convention de La Haye du 5 octobre 1961” is placed. This is written in French by article 4 of the Convention. The numbered fields contains the following information in two languages:
