What are haemorrhoids?

The dilatated veins in the anal canal and the distal region of the rectum are haemorrhoids, often known as piles.

What causes haemorroids?

Causes of Piles include: ageing, chronic constipation or diarrhoea, pregnancy, heredity, Faulty bowel function due to misuse of laxatives or enemas, strain during stool movements, sitting for a long time in the toilet. The tissues supporting the veins stretch, because of the causes. This causes a dilation of the veins, thinness of the walls and bleeding. The weaker veins will protrude, if the pressure and the strain persist.

What are the symptoms?

Bright red bleeding with defecation, sensitivity (severe pain when thrombosed), external protrusion, burning, itching, and soilage.

What do I do if I have hemorrhoids?

It is better to consult the doctor, as soon as you see the symptoms Your doctor should check that your piles are at which stage. Medications may be utilised in the early stages to treat haemorrhoids. In advanced cases, however, surgery may be necessary.

Grades of Piles

  1. Grade I hemorrhoids have not prolapsed.

You should use Natural remedies, and do some Lifestyle (diet) adjustments to cure Grade I piles.

2.                   Grade II hemorrhoids prolapse upon bearing down but reduce spontaneously when the pressure is relieved.

You can use Natural remedies and make some Lifestyle (diet) changes to cure Grade II.

Rubber band ligation (optional)

3.                   Grade III hemorrhoids prolapse upon bearing down and require manual reduction.

You can make Lifestyle (diet) adjustments to reduce the symptoms. And you can get Rubber band ligation, and Sclerotherapy to cure Grade III Piles.

4.                   Grade IV hemorrhoids are prolapsed and cannot be reduced manually.

The options for treatment of Grade IV piles can be either Sclerotherapy or Hemorrhoidectomy. In some cases, the combined procedures of Sclerotherapy and Hemorrhoidectomy can be done.

What are the options for Piles surgery in Bangalore?

  1. Rubber band ligation:

Rubber banding is a method whereby the hemorrhoid base is wrapped with rubber bands and the hemorrhoid is cut off. This treatment is for internal haemorrhoids.

A doctor inserts a viewing tool (anoscope) into the anus to do this procedure. A device is attached to the hemorrhoid and a rubber band is placed around the base of the hemorrhoid. The hemorrhoid then shrinks and dies and falls down in approximately a week. Instead of haemorrhoids, a scar will grow, holding close veins so that they do not bulge in the anal canal.

The treatment is performed in the office of a doctor. You will be asked if the rubber bands are feeling too tight. When the bands are really painful, they might be injected with a medicine into the banded haemorrhoids to reduce pain. You may feel pain and feel full in the lower abdomen after the surgery.

2.                   Sclerotherapy:

A proctoscope is placed into the anus gently in this operation. A proctoscope is a small tube with a light that may be used by the doctor to examine the membranes lining the anus. A liquid with medicines like quinine, polidocanol or zinc chloride is injected into the region surrounding the enlarged haemorrhoids using the proctoscope. This treatment is to reduce the blood vessels and the blood flow to haemorrhoids by shrinking the haemorrhoids. It is generally necessary to treat all the enlarged haemorrhoids many times. The injections usually take place every few weeks.

Some individuals suffer bleeding, mild pain, pressure, or the sensation of something in the back after sclerotherapy. This may also lead to swelling and infections in rare situations. Sometimes in the anal area there is a painful blood clot (thrombus). After two to three years, haemorrhoids again recur in many people.

3.                   Hemorrhoidectomy:

During such surgery, enlarged haemorrhoids are removed using tools such as scissors, scalpel or laser.  The incision is then partly or completely closed with stitches in certain techniques. These are called "open," "partly open," or "closed" surgical procedures. If the incision is left partially or fully exposed, there will be less stitch-related complications and less hematoma (bruising) issues . One disadvantage is that exposed wounds take longer to heal.

4- Stapling

Stapling is a relatively modern technology used in treating haemorrhoids of grade 3. In this operation, part of the swollen tissue is initially removed and the remaining tissue is "stappled" into the anus.

Those who undergone this surgery have less pain after surgery than those who get a hemorrhoidectomy. You might also return to daily activities quickly. However, stapling has a disadvantage too.  After the surgery the haemorrhoids can grew back again. Thus, it could be required to repeat the stapling process sooner in comparison with hemorrhoidectomy. Side effects such as itching or bleeding were equally prevalent after stapling as after other surgeries.
