HAIR TRANSPLANT SURGERY

Hair transplant is a surgical procedure for the restoration of natural
hair on the bald area. In this process, individual hair follicles are
removed from the donor site to balding part called the receipt area.
In this minimally invasive procedure, hair from the donor site, which
is genetically resistant to hair loss, such as the back of the head
and sides, are transplanted to bald scalp. This procedure can also be
used for the natural restoration of eyebrows, eyelashes, beard ,
chest, pubic hair, and to fill in scars caused by accident or surgery
such as face lift or the previous transplant. It is similar to skin
grafting but is different in the way that grafts extracted from the
donor sites contain all of the epidermis and dermis surrounding the
hair follicle and many grafts are transplanted rather than a single
strip of skin. This procedure is called Follicular Unit
Transplantation. Donor hair can be harvested in two different ways,
i.e. strip harvesting and Follicular Unit Extraction.

Transplant operations are performed after the diagnosis of underlying
diseases. If the underlying causes can be beaten back with the help of
drugs such as Rogaine, Propecia or Cortisone, hair restoration cannot
be opted as first option. Hair fall caused by certain dysfunctions
such as polycystic ovary syndrome (which is the inherited
dysfunctional condition affecting the ovaries), hyperthyroidism (a
sign of thyroid dysfunction), pregnancy, post facial plastic surgery
can be reserved to the normal hair growth after the dysfunction is
properly treated. Hair transplant surgery cannot restore hair on the
total bald head where there is no donor site to extract follicle
grouping. Patient facing partial baldness caused by some injury or
resulted from hormonal imbalance can be the candidate for hair
transplantation. Transplant operations are performed on an outpatient
basis, with mildsedation which is optional, and injected local
anesthesia, which typically last about six hours. Patient’s hairs are
shampooed and an antibacterial agent is applied on the scalp. There
are different methods of harvesting the hair follicles, each possess
specific advantage and disadvantages. However basic procedure is
almost same in all hair restoration techniques, the perfect follicle
extraction and the proper placement is the validity of the success of
surgery. The extracted follicle grows at a slight angle to the skin
surface that is why they must not be place perpendicular to the skin
surface.

There are two main ways in which donor grafts are extracted today:
strip excision harvesting, and follicular unit extraction. FUT or
strip harvesting, a strip of skin is dissected with a single, double
or triple-bladed scalpel from the posterior scalp, an area of good
natural growth. Each incision is planned so that intact hair follicles
are removed. The excised strip is about 1–1.5 x 15–30 cm in size. At
the time of harvesting, follicular unit grafts are dissected to
separate each follicular unit. With the help of stereo-microscope,
additional fibrous and fatty tissues are removed to avoid the damage
of follicular cells that will be used for grafting. The latest method
of closure is called 'Trichophytic Closure' which results in much
finer scars at the donor area. FUE-Follicular unit extraction is
different from the strip harvesting in way that instead of removing a
strip of skin, only follicular units are extracted from donor area and
