Who Should Take a Refresher Course in Scuba Diving?

What Must You Bring with You

The Scuba Fort Worth instructor requires you to bring a few items with you on the day of your PADI training. Could you please fill out the PADI paperwork and bring them with you on the day? Bring your PADI certification card if you have one. You can get a new certification card on the website www.padi.com if you lose yours. If you notify PADI that you have lost your card, they will guarantee that you receive a replacement certification. You should also include your swimsuit and diving logbook.

Please remember: If you answered 'yes' to any of the questions on the medical statement, we would need a note from your doctor stating that you are okay to dive.
