This tool, which is now mandated in Maryland for anyone seeking care, can seem daunting, but, as CHES explains, it is actually extremely beneficial for you and your loved one with a disability.

Rather than assessing the level of their disability, the HRST measures your loved one’s health risk. The purpose of the tool is to identify health risks early and prevent future complications and identify service needs that may be less obvious. As such, if done correctly under the guidance of one of the agency’s CM or DN nurses, it can allow for greater ongoing care assistance and monitoring, and can ultimately lead to improvements in the quality of life of your loved one.

The HRST is just one of many assessments that CHES’ CM and DN nurses oversee.
