Abstract

Paroxysmal nocturnal hemoglobinuria is a rare blood disorder which is characterized by the premature breakdown of the red blood cells. This earlier than usual destruction of red blood cells leads to development of various signs and symptoms and complications like discoloration of urine to major one like stroke or leukemia. Generally no risk factor is associated with it and the main cause behind it is the gene called PIGA but this disease is not an inherited one. Introduction

Paroxysmal nocturnal hemoglobinuria is an acquired genetic disease which however is rare in occurrence. Mutations in genes result in loss of PIGA gene which leads to the immune system attacking on red blood cells and their premature breakdown. These premature red blood cells lack the proteins which protect them. An individual of any age can get affected by this disorder. No one is born with it and this disorder can result in really deadly conditions if not managed timely.

Due to loss of PIGA gene there occurs mutations in hematopoietic stem cells. Hematopoietic stem cells are present in bone marrow which are responsible for the production of the blood cells and mutation in these cells lead to abnormality in the blood cells as well. Usually there is a layer of protein on red blood cells which signals the immune system that they are not foreign to the body but when this layer is absent (due to absence of PIGA gene) the immune system recognizes it as foreign body and destroys them. In PNH the platelet count is also low which results in the impaired functioning and difficulty in clotting blood.

Signs and Symptoms

The early sign of PNH is blood in urine as when premature red blood cells are destroyed it leads to release of hemoglobin into the urine giving it red color. Usually this discolored urine is passed at night or in morning as during this time the urine is well collected into the bladder for a long time. Along with it, there can be several other signs and symptoms as well like.

• Pain in abdomen
• Shortness of breath
• Easy bruising on body
• Heartbeat is irregular
• Difficulty in swallowing
• Erectile dysfunction in men
• Skin appears to be pale or yellow in color

Causes

The main cause behind the occurrence of PNH is an acquired somatic mutation in the PIGA genes which results in defects in the hematopoietic cells causing early breakdown of the red blood cells. During this whole pathological condition when the genetic mutations occur the abnormal red blood cells keep on proliferating while the healthy ones are destroyed early by the immune system of the body. PNH is not transferred by parents to children but it is a somatic mutated disorder which means the mutation which occurs after the conception. This mutation occurs randomly and there is no obvious cause or age factor behind it.

Diagnosis

Medical history taken by your doctor will help in initiation of the diagnosis of PNH. Certain signs and symptoms like blood in urine, anemia, unexplained blood clots, easy bruising with these complaints are suggestive of PNH. However for the confirmation of the diagnosis flow cytometry analysis of red blood cells is done. It is the best and most specific test to confirm the PNH.
