Caring For IDD Patients

CHES is comprised of experienced IDD nurses who make every effort to ensure that each patient is comfortable and cared for. Specifically, they are trained to notice behavioral nuances, which may suggest a change in the client's condition. For example, people with Prader-Willi syndrome have a higher risk of breathing disorders, and IDD nurses take special care to notice any behaviors that may be masking a health condition, such as head-hitting or unexplained frustration.

How Self-Directed Care Works

Individuals with IDD who are eligible for self-directed care based on their diagnosis and psychological reports can now choose and manage their home care providers directly or through an authorized representative.

Community Health and Education Services provides in-home personal care and self-directed nursing services for adults and children with IDD.

All plans of care are designed to help patients achieve their daily living goals. Additionally, IDD nurses at CHES regularly review these plans to ensure patients continue to receive the appropriate support.

The Scope of Self-Directed Care

CHES services are designed to cater to both children and adults with developmental disabilities in Montgomery County, Prince George's County, and the surrounding areas. Operating under the Community First Choice initiative of Medicaid, CHES allows Medicaid participants to access important self-directed home-based care services.

Skilled nursing is a key component of CHES's updated in-home care services. Registered Nurses oversee or serve as visiting nurses, managing cases and providing a range of medical services, including nutrition monitoring, vitals assessments, wound care, post-surgical care, diabetic care and education, and pain and symptom management to improve clients' comfort levels.

Another important aspect of CHES's new at-home services is personal care. Certified personal care assistants assist clients with daily tasks such as bathing, eating, light housework, and mobility exercises. They also provide support with meal planning, grocery shopping, medication reminders, and status reporting to clients' families.

By working closely with clients and their primary care providers, CHES can adjust self-directed care plans to best serve the client's interests.
