Herbal Remedies for Esophageal Motility Disorders

Abstract

Esophageal motility disorders are a group of disorders in which there is a problem in movement of food from the oesophagus to the stomach. The efficient supply of food through oesophagus required its proper mechanical functioning but in esophageal motility disorder this mechanical functioning gets hampered. In this article we are going to discuss what esophageal motility disorder is and what is the cause of this disorder along with its various symptoms. Also we are going to discuss the ayurvedic aspect of this condition along with its successful ayurvedic management by planet ayurveda.

Introduction

Esophageal motility disorder is a medical condition that can occur due to malfunctioning of the oesophagus which affects the motility of the oesophagus and hinder the food supply up to the stomach. It is of two types on the basis of its cause that are primary and secondary. Types and Causes of Esophageal Motility Disorders

These are of Two Types on the Basis of Causes

Primary Esophageal Motility Disorders: It occurs due to any abnormality in the oesophagus itself.

  • Achalasia: It occurs when nerves present in the oesophagus get damaged. The damaged nerves get paralysed and become dilated for the time remaining. Due to this the difficulty occurs in squeezing the food down. This condition is associated with the symptoms like difficulty in swallowing food along with heartburn, belching, coughing at night, weight loss, vomiting etc.
  • Distal esophageal spasm: It is considered as any of the problems  in the muscles of the oesophagus that can cause contractions in the muscles and make it harder to swallow the food and it is quite painful.
  • Hypertensive LES (lower esophageal sphincter): Here the pressure increases in the lower esophageal sphincter area that is more than 45mmHg which makes it difficult to pass food from the lower end of the oesophagus to the stomach.

Secondary Esophageal Motility Disorders: These conditions occur secondary to any other condition of the oesophagus or any disease that may affect the oesophagus not directly correlated to the oesophagus.

1. Secondary Conditions To the Oesophagus

It can be intrinsic which affects esophageal motility from inside and extrinsic that affect the motility from outside. Intrinsic ones are- carcinoma, benign tumours, foriegn body, peptic stricture, etc. And the extrinsic ones are spinal osteophytes (bony growth in the spine that pressurises the oesophagus), mediastinal mass, vascular compression (any vessel compressing the oesophagus) (any vessel compressing over the oesophagus).

2. Diseases Related Secondarily

These conditions are generally those in which muscles of the oesophagus get affected and lead to failed motility. Some of the conditions are mentioned below

3. Muscular Dystrophy

In this disease there is deficiency in muscles so this can lead to improper movement of muscles

4. Myasthenia Gravis

The condition generally affects the contraction of muscles so it may affect the contraction of oesophagus also.

5. Parkinson Disease

As the patient is generally inactive so this may lead to muscle wasting and muscle weakness.

So all such conditions that can lead to muscle weakness are considered under the conditions that can lead to hampered esophageal motility.

Ayurvedic View of Esophageal Motility Disorders

As per ayurveda this condition is primarily considered under vata dosha vitiation as hampered motility is the characteristic feature of the esophageal motility disorders. Along with that the condition is associated with pitta and kapha dosha vitiation also. As we see that in this condition there is dysphagia, spasm of muscles, chest pain, and regurgitation so all these symptoms are due to vitiated vata dosha. Heartburn is due to pitta dosha imbalance and recurrent pneumonia is due to kapha dosha elevation occurring due to stuck food in the upper end of the esophagus.

As per ayurveda this condition is considered under vata pradhan (predominating)  tridosha. So the treatment option for esophageal motility disorders is to balance all the doshas-vata, pitta and kapha. The main treatment is vata anulomana (removing vata by downward movement from the anal region) and muscle tone reduction. Along with doing this treatment we have to balance pitta and kapha dosha also. PRODUCT DESCRIPTION

1. Avipattikar Churna

This is a classical herbal formulation made by several ingredients such as pippali (Piper longum), shunthi (Zingiber officinale),marich (Piper nigrum), amalaki (Emblica officinalis), haritaki (Terminalia chebula), Bhibhitaki (Terminalia bellirica), nagarmotha (Cyperus scariosus)  and some other herbs. All these ingredients are finely powdered and mixed together and formulated as churna. As we read in the above study, vata anulomana is the first most treatment we have to do in esophageal motility disorder. Vata anulomana means expulsion of vata dosha towards the downward route by doing so this churna helps in promoting the movement of food from the oesophagus towards the stomach that is stuck in the upper end of the oesophagus.

Dosage: 1 teaspoon twice a day after meals with plain water

2. Mahashankh Vati

This formulation is also a classical formulation that is present in tablet form which includes ingredients such as shunthi (Zingiber officinale),  pippali (Piper longum), shankha bhasma (calyx of conch shell), chincha shaal or tamarind bark (Tamarindus indica), Shuddha Vatsanabha (Aconitum ferox), saindhava lavana (rock salt), Sauvarchala Lavana (black salt) and few other ingredients. As we know that symptoms of pitta dosha aggravation are also associated with this condition so pitta dosha also needs to be pacified. So for pacifying pitta dosha mahashankh vati is given as the ingredients present in this vati helps in balancing pitta dosha and helps in lowering the symptoms of esophageal motility disorders in a helpful way.

Dosage: 2 tablets twice daily before meals with plain water.

3. Digesto Plan Syrup

This formulation is available in the syrup form and includes standardised extracts of Triphala [amalaki (Emblica officinalis), haritaki (Terminalia chebula) and Bibhitaki (Terminalia bellirica)], trikatu [shunthi (Zingiber officinale), marich (Piper nigrum), pippali (Piper longum)], ajwain (Trachyspermum ammi), jeerak (Cuminum cyminum), dhaniya (Coriandrum sativum), saunf (Foeniculum vulgare) and some other ingredients. The syrup promotes digestion by improving the peristaltic movements of the GI tract. Also it helps in the absorption of food after digestion by enhancing the digestive enzymes secretion. Along with that it pacifies vata dosha to promote balancing of agni to induce good digestion.

Dosage: 2 teaspoons twice a day after meals with plain water.

4. Acidoplan Syrup

This syrup formulation is also present in the form of syrup that contains patola (Trichosanthes dioica), sajji kshar (soda-bicarb), ghrit kumari (Aloe barbadensis), saunf (Foeniculum vulgare), kokum (Garcinia indica), Pitta Papada (Fumaria indica), shatavari (Asparagus racemosus) and triphala [amalaki (Emblica officinalis), haritaki (Terminalia chebula) and Bibhitaki (Terminalia bellirica)]. As we studied earlier that the symptoms of esophageal motility disorders also include heartburn, regurgitation and vomiting, all these mainly occurs due to pitta dosha vitiation and these vitiations cause all these symptoms so it is must to balance pitta dosha to get rid of these symptoms. So this syrup helps in curing these symptoms by pacifying the pitta dosha.

Dosage: 2 teaspoons twice a day with plain water after meals.

5. Digestion Support

This herbal remedy is present in the form of capsules. And these capsules contain- These capsules contain amalaki (Emblica officinalis) , haritaki  (Terminalia chebula) , bibhitaki (Terminalia bellirica) , saunf (Foeniculum vulgare) , dhaniya (Coriandrum sativum) , jeerak (Cuminum cyminum)  and pippali (Piper longum) . These ingredients are present as standardised extracts in these capsules. These capsules are very effective in relaxing the smooth muscles and relieves uncomfortable muscle spasms.
