Combined Medical Services 2015 Application form

Combined Medical Services is an recruitment exam for the recruitment of candidates as medical officers in various government offices. Candidates will have to appear in the exam and qualify it to get recruited. To apply in the exam candidates will have to fill the CMS 2015 Application Form otherwise they will not be able to appear in exam. Candidates will be selected on their performance in written exam and interview round as well.
The form will be released on 14 March 2015. Other details for the application form are given below in the article.

Date of Release: 14 March 2015 and the last date to submit CMS forms is 10 April 2015.

The candidates those who want to appear in the exam will have to first make sure that they are eligible for the exam. For that they must check themselves on the eligibility conditions. If they fulfill all the eligibility conditions only then they are eligible for filling the CMS 2015 Application Form. If any ineligible candidate fill the application for the exam then his/ her application will be rejected without any return of application fees.

Application Process

Candidates can follow the steps given here to apply for the exam. These steps are easy so that each and every candidate can easily follow them.

1. Visit the website of UPSC as form will not be released via any other mode.
2. Read all the instructions before filling up the application form.
3. The form will be completed in two parts.
4. In part 1 they will have to fill all the details correctly and completely.
5. If any field is remain unfilled or if the details entered by them are incorrect then the application will be rejected.
6. Upload the photograph and signature.
7. Recheck the details and submit the form.
8. In part 2 they will have to pay the application form fees using one of the available modes.
9. Take print out of the form and fees receipt as well.

CMS 2015 Application Form Fees

Candidates will have to pay the fees for CMS 2015 Application Form otherwise the form will not be considered complete. They will have to pay 200/- as application fees. It can be paid using credit or debit card. It can also be paid using challan form and  via cash as well.

• Dimensions of the images that need to be uploaded in the form is as follows:
• Images must be in JPEG format.
• Size of the photograph 3kb to 40kb.
• Size of the signature 1kb to 40kb.
• Both images must be clear, contrast and legitimate to view.

Candidates those who qualify the written exam will have to submit the required documents. These documents are:

• Age certificate;
• Education qualification certificate;
• Category certificate;
• Proof for claiming fees concession;
• Other certificates.

Other Instructions

While filling the CMS 2015 Application Form candidates must consider various other points to avoid any kind of mistake:

• A candidate must submit only one application form. More than one application form will not increase their chances for selection.
• If in case candidates need to fill another application form then the application for with higher application number will be considered.
• The candidates those who are already employed will have to submit an undertaking in writing.
• All the required copies must be submitted at the time of document verification. The documents must be attested as well.
