Abstract

Eruptive syringoma is a variant of syringoma which is very rare in occurrence. This benign adnexal tumor affects the eccrine sweat ducts, specifically the intraepidermal portion. Eruptive syringoma affects the chest, neck, arms and other anterior surfaces of the body. Individuals in puberty are usually reported to be affected with this disorder. Introduction

Syringoma is a well formed and differentiated benign adnexal neoplasm with ductal elements. Eruptive syringoma is one of the types of Syringoma and is a non cancerous, benign growth caused due to sweat glands over activity. They usually grow on the lower region of eyes, upper cheeks or neck, however abdomen, scalp, armpit, genitals and belly button are also the sites of appearance. Eruptive syringoma is one of the types of syringoma, while other being familial, multiple and associated with Down syndrome.

Usually the Eruptive syringomas are harmless and do not cause any complaints however, rarely there are reported cases of itching and pain while sweating on the eruptions. In rare cases genetic predisposition is also seen in the individuals developing eruptive syringoma. People usually develop them at the age of 25-30 years, i.e. early adulthood. Several medical disorders are also found associated with it like diabetes.

Eruptive syringoma

Eruptive syringomas are firm bumps or small papules that are approximately 1-3 mm wide and grow in small groups. These eruptions are yellow, pale pink, brown or skin toned in color. The same pattern of the eruptive syringomas appears on the body's both sides, i.e. symmetrical. This disorder can be confused with other similar conditions like Xanthoma, milia, lichen planus, sebaceous hyperplasia, flat warts, acne vulgaris or basal cell skin cancer. Hence the correct diagnosis is necessary by an expert. Eruptive syringomas appear suddenly in individuals however the other types of disorder develop gradually over the time.

Certain people are at high risk of getting them as compared to others, like people having

  • Ehlers-Danlos syndrome
  • Diabetes mellitus
  • Marfan syndrome
  • Down syndrome

Pathogenesis of the eruptive syringomas is completely based on the sweat glands. These tubular structures or sweat ducts carry sweat to the surface of skin from sweat glands for cooling the skin and releasing it. Over activity of the sweat glands or abnormal growth of the sweat duct cells in the skin’s outermost layer results in formation of the abnormal tissue or tumor.

One of the differentiating points of eruptive syringoma from other types is also that in this type the individual has the intermittent phases of relief in between the two active phases of rash. Eruptive syringoma is considered to be a reactive hyperplasia which is usually activated after the eczema-like inflammatory condition of the skin. Also there is female predominance in this disorder which suggests the involvement of the hormones in its pathogenesis.

Diagnosis

As already known, eruptive syringomas can be misdiagnosed as many other similar conditions of the skin, hence it is necessary to diagnose it correctly. Usually the qualified and specialized doctor can easily diagnose it on the basis of clinical presentation, lack of associated signs and symptoms, pattern of the appearance of the eruptions and locations and family history. Further diagnosis can be confirmed by skin biopsy in which the tissue is taken from the affected area and examined under a microscope. In this there are characteristic comma-shaped tails of cystic and dilated eccrine ducts.

Treatment of Eruptive Syringomas

Usually the syringomas do not need any treatment as they do not cause any harm to the person but cosmetically they can harm the appearance of the face or the place where they appear which can be the reason for people to go for the treatment. Conventionally, syringomas are treated through two ways, i.e. medications or surgery. In medication there are given some trichloroacetic acid to be applied topically which makes them fall or few oral medications can also be given. Surgical approaches include laser removal, electric cauterization, cryotherapy, electrodessication with curettage, dermabrasion, and manual excision.

However all of these ways can be painful to some extent and also act as temporary relief to the condition.
