The initial assessment of male infertility consists of carrying out a precise interrogation, a directed physical examination, and a program. To really speak of azoospermia, there must be an absence of sperm in the program on two examinations with centrifugation three months apart. Centrifugation of the sperm makes it possible to distinguish between azoospermia which is a true absence of sperm and cryptozoospermia which is a situation where there are only a few rare sperm in the semen.

For humans, azoospermia is asymptomatic. Trying to have a child for a year without contraception defines childlessness. The infertility of the couple is of male origin in 20% of the cases and mixed in 40% of the cases. Men are therefore at least partially responsible for infertility in 60% of cases.
