Overview of TAVI/TAVR

Transcatheter Aortic Valve Replacement (TAVR) or Transcatheter Aortic Valve Implantation (TAVI) is a minimally invasive procedure used to replace the aortic valve in patients with severe Aortic Stenosis, without removing the old damaged aortic valve.

The aortic valve is a one-way check valve that allows blood to be pumped out from the left ventricle of the heart to all vital organs and other parts of the body. The aortic valve may become narrow due to aging which affects the blood supply to the body. Due to severe Aortic Stenosis, inadequate quantity of blood is pumped into the circulation.

As a result, patients usually experience breathlessness, chest discomfort, and fainting spells. Untreated Aortic Stenosis can lead to more serious disorders, including heart failure and may eventually lead to death. The patients of Aortic Stenosis who are considered to be at high risk for surgery are treated well with this advanced TAVI/TAVR Surgery in India as well.

How is TAVI/TAVR performed?

TAVI / TAVR surgery procedure is performed in a hybrid room having facility of operating room and cath lab capabilities. The procedure is usually performed under direct visualization with fluoroscopy by a team of an interventional cardiologist, cardiac surgeon, and anesthesiologist.

However, occasionally the procedure is done with Transesophageal Echocardiogram (TEE) guidance as well. TAVI / TAVR procedure is done through a relatively small tube called catheter that is fitted with a compressed TAVR valve.

The catheter containing an artificial valve is then passed through the Femoral artery by giving a small incision in the groin up to the existing diseased aortic valve in the heart. TAVI / TAVR valve is expanded into its working size pushing the old valve out of the way. The tissue in the replacement valve takes over the job of regulating blood flow.

How you suspect TAVI/TAVR from your symptoms?

Aortic Stenosis develops gradually, silently and invariably, noticeable symptoms do not appear until blood flow is significantly reduced.

Symptoms of Aortic Stenosis include:

  • Breathlessness
  • Chest pain or tightness in the chest,
  • Syncope i.e. fainting
  • Palpitations - felt like heavy, pounding and noticeable heartbeats.
  • The decline in activity level
  • The reduced ability for normal activities
  • Tiredness with mild exertion.

Untreated Aortic Stenosis often develops into heart failures and can cause sudden death.

How TAVI/TAVR is different from the surgical treatment of valve replacement?

Surgical replacement of aortic valve has been the standard treatment for patients with severe symptomatic Aortic Stenosis. Earlier, patients suffering from severe Aortic Stenosis that were considered to be at high risk of surgery were given a palliative treatment with diuretics and balloon valvuloplasty but with limited outcomes.

The advent of Transcatheter Aortic Valve Replacement (TAVR) has surfaced as a lifeline for inoperable patients. In conventional open-heart surgery, the old aortic valve is removed and replaced with a new valve.

In the TAVI / TAVR procedure, the old valve is not removed but the new valve is placed and expanded inside the existing old valve to simply crush it out of the way.

Who is a good candidate for TAVI/TAVR?

The patients who suffer from Aortic Stenosis but inoperable, being at high risk for open-heart surgery, are good candidates for TAVI/TAVR.
